1. Download the Application for Admission Form (nwu-SHS.001) from the links at the bottom of this page and print the form on size A4 bond paper. Accomplish the form completely and submit the same together with the following requirements.

a.    Photocopy of your final, completed Grade 10 report card SY 2015-2016.

b.    Two recent I.D. pictures (1 ” x 1 ”) in COLOR

c.     Photocopy of Birth Certificate (NSO Copy)

d.    If applicant is NOT a Filipino citizen, a photocopy of your Passport and Alien Certificate of Registration (ACR I-Card).

e.    If applicant has DUAL citizenship, submit a photocopy of your Philippine Passport or a Certificate of Recognition as a Filipino Citizen.

2. Pay the amount of Php500.00 admission fee at the finance office.

*For expatriates and repatriating Filipino applicants from foreign schools, an additional processing fee of US$25.00 will be charged.

3. Present your (a) Official Receipt, (b) fully accomplished Application Form, (c) a complete set of requirements to the Senior High School department head.
